One of the requirements for our Council to become qualified yet again as a Star Council of the Order for our 13th consecutive year is to submit a Council Survey of Fraternal Activities. This is also critical for the Order to maintain its 501.c3 status. Many of the program reports funnel information into this annual report. However, they do not necessarily capture the multitude of visits completed and volunteer hours donated by individual Knights!
To help with this compilation the Individual Member Worksheet (Form 1728A ) is provided here.
All members of the Council are requested to download and print this form, fill it out and submit to the Grand Knight, Deputy Grand Knight, Financial Secretary, or put it in the collection box at the Welcome Desk in the Commons of the Church.Submissions should be made as soon as possible, but not later than December 31, 2016 to enable the Financial Secretary to compile the full Council Survey.
Instructions are provided on the top of the form for the Financial Secretaries, Comptrollers, and individual members. You will note that below the instructions the form has two identical form sections. As noted in the instructions a separate form must be submitted for the Council and Assembly. All members should fill out one of the form sections and submit it to the Council. The other form section is provided as a convenience for Fourth Degree members to fill out and provide separately to the Father Habets Assembly 1505, for any activities that were specific to the Assembly. DO NOT DUPLICATE visits or volunteer hours on both forms; they must be separate and distinct, with each indicating only what was done for that particular organization.
To further help you to accurately fill out your individual survey the following suggestions, keyed to the question numbers, are provided:
1. Just count and list the number of VISITS, irrespective of their length. A visit consists of one arrival and one departure. If you visit a sick person in the hospital every day for a week, but go home every day after the visit, that is 7 visits. If you call on a family at the funeral home it is one visit; if you then attend the funeral on a different occasion, or day, it is another visit.
2. A blood donation consists of one pint. Count the number of pints donated during the year and enter that number.
3. This is where we enter the number of HOURS.
a. CHURCH ACTIVITIES : include hours served as Extraordinary Minister, Lector, carrying communion to the sick and infirmed, ushers, Saint John prayer chain, odd jobs for church building upkeep, bereavement ministry, bereavement hospitality, donut ministry, welcome committee, greeting committee, setup and takedown of chairs for church in the school gym, CHRP, KCIC card sales, Fifth Sunday rosary, RCIA participation and support, catechist/aide/helper , baptism ministry, First Penance and Communion preparation, Confirmation table monitoring, choir, cantor, song leader, bible study leader/moderator4. HOURS OF FRATERNAL SERVICE : report hours here for the visits previously reported in item #1. Focus is hours for sick/disabled members and their family members (including your own family), helping with household chores, providing transportation, tutoring members and their family members, welfare visits
b. COMMUNIUTY ACTIVITIES : include hours served on Parish picnic, meals on wheels, sandwich brigade, holiday outreach, Hope Haven, Seton Youth Shelter, Samaritan House, Sunday nursery, vacation bible school, parish council or committees, World Wide Marriage Encounter circle, parish social events/committees, hours in support of any SJA school support activity (e.g., unpaid volunteer work, PTA, golf classic, gala, school clubs, etc.), wheelchair program, support our troops, election poll worker, planning and working on blood drives, hours working on parish breakfasts
c. YOUTH ACTIVITIES : hours spent on Soccer Shootout, judging essay contests, judging KCIC and Drug & Alcohol Awareness poster contests, Free Throw Shootout, Altar Server picnic, middle school youth group, high school youth group, chaperone youth parties/dances/campouts, boy scouts, girl scouts, youth mentoring program, school youth sports coaching, school youth sports event support,
d. HABITAT FOR HUMANITY : council did not have any sponsored activities, report any individual hours donated to Habitat projects
e. CULTURE OF LIFE ACTIVITIES : hours spent on 40 days for life participation, praying the Pro-Life rosary for an end to abortion, March for Life attendance, Hope 4 Life support, annual Life Chain event, baby bottle drive,
f. VETERANS ADMINISTRATION VOLUNTEER SUPPORT (VAVS) : hours spent on VAVS birthday parties, hours spent volunteering at VA hospitals, hours spent assisting veterans or their immediate families in any manner
g. FOOD FOR FAMILIES : hours spent planning and preparing for food drives, hours collecting foodstuffs, hours sorting and preparing deliveries to individuals and food pantries, hours delivering food supplies to recipients (to food panties/banks, Thanksgiving food baskets, Christmas food baskets).
h. SPECIAL OLYMPICS : hours spent soliciting donations for Special Olympics, for Special Olympics Polar Plunge, time spent doing polar plunging, serving meals to Special Olympians at the Fall Championships, assisting at event venues for fall championships, assisting in organizing Special Olympics ceremonies (opening, closing, etc.), participation in Special Olympics ceremonies.
i. MISCELLANEOUS ACTIVITIES : hours spent on anything not otherwise mentioned above. Council website creation/maintenance, preparation of website inputs and updates, hours volunteering at Strawberry Festival parking project, time served on Council special committees, time preparing for and presenting degree exemplifications, time preparing and hosting family nights and council parties and special events
Please write/print legibly. Record zeros for any line for which you had no contribution of visits or hours. We need a form from every member, no matter how small the number of visits or hours contributed. Please print the form, fill it out, and submit as soon as possible but not later than December 31, 2016 as this will help in consolidating potentially 300 plus inputs by the mid-January before the deadline.
